Partnerships and Acknowledgements
In creating an accessible service, Integrated Family and Youth Service recognises that it is crucial to be involved in partnerships with external organisations in order to better assist children, young people and families remain connected to their community of choice.
We believe in
- WORKING TOGETHER. Working collaboratively with communities and agencies to maximise support to young people and their families and ensure that co-ordination of assistance occurs.
- CLIENT CENTRED / CLIENT DRIVEN CASE MANAGEMENT that recognises the importance of the initial intake and assessment process. Our experience of working with clients for a number of years has reinforced the need to respond in a timely and professional manner. In a culture where asking for help is discouraged, it is critical to ensure that when people do ask they feel supported, accepted and affirmed in their request for support.
- VALUING AND RECOGNISING THE NEEDS OF YOUNG PEOPLE AND FAMILIES and the way they access services can be different for each individual. Integrated Family and Youth Service seeks to promote services and partnerships that maximise quality of information in a way that reaches both young people and families.
- CLEAR AND CONSISTENT REFERRAL PROCESSES. Our experience is that many clients fall between services and become isolated from support as a result of poor referral processes. A referral is more than a phone number and if done in a professional and supported manner then the pathway to client support is most effective.
- COMPREHENSIVE SERVICING. Providing or facilitating access to the range of services that can best contribute to positive outcomes for young people and their families.
