Inclusive Living Program

The Inclusive Living Program provides flexible and innovative support services by working with people experiencing a disability and their families. ILP provides brokerage or direct delivery of a range of services that maximise the service users’ opportunities to be self-determining and enjoy an improved quality of life. Currently IFYS provides support to a number of service users within their own homes throughout the Sunshine Coast and Rockhampton.

The IFYS Inclusive Living Program is a flexible and innovative disability support program which has a commitment to the following principles:

Persons with disabilities have an inherent right to respect for their human dignity and the same fundamental rights as fellow citizens of the same age.

The United Nations Declaration and Disability Service Act (2006) confirms that people with disabilities have the right to:

· Live in and be part of the Community.
· Make decisions that affect their lives.
· Realise their maximum capacity for growth and development.
· Protection from exploitation and discrimination

Everyone is of equal value as a person and has the right to live a lifestyle of his/her choice and therefore people with disabilities and their families should have access to the information they need to make informed choices about their lifestyles and the type of services they require.

The purpose of the Inclusive Living Program is to maximise opportunities for self-determination, to assist the individuals to enjoy an improved quality of life and to support their families (where appropriate) in their caring role.

IFYS is responsible for coordinating Three (3) disability programs:-

· The Family Support Program;
· The Adult Lifestyle Support Program;
· Individuals in Residential Aged Care Program;

The Family Support Program is aimed at assisting individuals with disabilities in maintaining and developing positive family relationships, providing direct assistance to support the families in their caring role and to assist the individuals in the development of positive lifestyle skills.

The Adult Lifestyle Support Program provides assistance to people with disabilities to improve their quality of life including support to maintain living arrangements of their choice and to participate in community life.

Young People in Residential Aged Care Program provides supports to people with a disability under the age of 60 who are inappropriately living, or are at risk of living, within aged care facilities and wish to live and participate within the community of their choice.

The Inclusive Living Program will work with Individuals, between the ages of 12-60 with a disability residing in the Sunshine Coast Region, Gold Coast, Cooloola, Pine Rivers, and Caboolture Local Government Areas.

The Inclusive Living Program Services will include:

· Practical support, including transport support
· Personal support, providing individuals with opportunities for skill development, positive self expression and building individual capacity and resilience
· Recreational and experiential learning opportunities
· Educational support
· Family support
· Community access and participation, including providing opportunities for positive peer connection
· Accommodation support
· Information and referral service
