Lifelong Planning Support for People with Disabilities

About the Program
The Lifelong Planning Support Program provides assistance to community members with disabilities, their families and carers during high risk transition points or during various significant life changes. The program connects community members with supports and services geared towards empowerment, community engagement and increased independence.
Transitions or life changes may include, but are not limited to:
- Finishing school
- Moving to a new school
- Starting a new job
- Commencing further study
- Entering the workforce
- Moving in or out of supported accommodation
- Engaging with the local community
- Engaging with community agencies
- Engaging with Government agencies
- Re-engaging with support agencies
The Lifelong Planning Support Program can involve a simple, one-off consultation providing information about community agencies, short term support based on individual situations or more intensive support for more complex issues. The latter may enable the officer to explore the full range of formal and informal supports available.
The Lifelong Planning Support Officer may work with the community member themselves, their families or carers. The officer may also identify opportunities to enhance the capacity of local communities or organisations to include people with a disability. The Lifelong Planning Support Officer has a wealth of knowledge about government, non-government and local community organisations that can assist the community member to achieve their life goals.
Eligibility
Community members up to 65 years, who identify themselves as having a disability (community members do not necessarily have to be registered with Disabilities Services Queensland to access this service).
Community members can self refer or be referred by parents or carers.
This service is currently offered to community members in the areas bounded by Peregian in the north of the Sunshine Coast, Beerwah to the south and Maleny to the west.
